WAIVER OF LIABILITY

I, _________________________________________, understand that myself and my dependents, ___________________________________ and ____________________________________ working as a volunteers to Volunteers of Love & MIA InnerLight project will expose (me/my dependents) to risks of injury or illness. I understand and assume these risks and agree not to hold Volunteers of Love & MIA InnerLight or its board of directs, or volunteers liable for such injury or illness. I certify that I am 16 years of age or older. (Minors must be accompanied by an adult 21 and over with Parental Permission Form).

I further certify that it has been recommended that I wear a comfortable clothes, gloves & eye wear on the work site. I understand that Volunteers of Love & MIA InnerLight Spiritist Society is not responsible for lost or stolen personal articles.

PHOTOGRAPHIC RELEASE

Volunteer does hereby grant and convey unto Volunteers of Love (VOL) & MIA InnerLight Spiritist Society (MIA) all rights, title, and interest in any and all photographic images and video or audio recordings made by/for VOL & MIA during the Volunteer's work for VOL & MIA, including but not limited to, any royalties, proceeds, or other benefits derived from such photographs or recordings. Images, video or audio recordings may be used for promotional purposes including but not limited to brochures, press releases, or social media outlets.
